PURPOSE This is a comparative study using secondary data from the Korean national long term care (LTC) insurance. METHODS Visiting nursing (VN) service users (n=666) and non-users (n=4,375) were extracted and compared in terms of medical expenditures, length of hospitalization, and annual number of ambulatory care visits to investigate effects of VN services in LTC. RESULTS Total health care expenditures were compared between the two groups and it was found that VN service users spent about $ 1700 than non-users for their medical costs between 2009 and 2011. The average length of in-hospital stay for VN service users was 19.4 days shorter than that of non-users. However, using VN services did not significantly influence the annual number of ambulatory care visits. CONCLUSION The study has found that VN services are effective ways of providing community-based LTC services. We recommend LTC policy makers to further utilize VN services to deliver cost effective health care services.

PURPOSE The purpose of this study was to demonstrate the effects of visiting nursing services in long-term care (LTC) insurance on changes in health status and physical functions among the elderly. METHODS We analyzed survey data on the living status and welfare needs of the elderly from 2008 and 2011 using the difference-in-difference method with propensity score matching. RESULTS The subjects were 76 elders including 38 visiting nursing service users(intervention group) and 38 home-based LTC service users (control group). Results from DID analysis confirmed that the subjective health status level of the intervention group was 2.2 points higher (p=.044) and the depression level was 0.3 point lower (p=.039) than the control group. In addition, the intervention group's ADL score was 1.9 (p=.027) and IADL 3.9 (p=.030), showing that their health was deteriorated less. CONCLUSION Visiting nursing service in LTC insurance was associated with delayed deterioration of subjective health status, depression, ADL and IADL. These findings suggest the need of rebuilding visiting nursing service programs focusing on prevention services, which will be more contributive to elderly health care and the reduction of social costs.

PURPOSE The purpose of this study was to examine satisfaction with counseling in long-term care service, and to compare the scores of counseling satisfaction according to variables among beneficiaries of Korean long-term care services. METHODS Questionnaires were completed by 445 beneficiaries of long-term care insurance to measure satisfaction with counseling. Research design was cross-sectional descriptive design. Data were analyzed using descriptive statistics, t-test and ANOVA for evaluating differences in satisfaction with counseling according to variables including economic status, the level of long-term care insurance approval, duration of long term care service, and conditions of counseling. RESULTS The score of satisfaction with counseling was somewhat high as 71.67. The score of counselor's attitude was highest among the subcategories of satisfaction. The factors that influenced satisfaction with counseling were frequency and time of counseling (F=12.19, p<.001). CONCLUSION Home-based individual counseling is necessary for the elderly who need long-term care service. The National Long-term Care Insurance Corporation should offer counseling and assistance to elders and their caregivers about long term care insurance.
PURPOSE This is a qualitative study to identify dimensions of long-term care hospital care quality that provide high-level medical services for long-term care patients in Korea. METHODS Service consumers and providers were interviewed, and collected data were analyzed into thesis, type and dimension. The focus group method was applied to two provider groups and individual interview was applied to two persons who had experienced a long-term care hospital. RESULTS The results of analyzing the comsumers and providers was integrated into 8 dimensions: physical environment, staff, clinical care and nursing, multiplicity of activity program, atmosphere, interaction with family, nutrition, and quality improvement system. CONCLUSION The dimensions of long-term care hospital care quality from this study can be used as a basis of quality indicators. Quantitative studies to test these dimensions are required for establishing quality management systems.
PURPOSE To analyze the home care services provided to the elderly aged 65 and older by a hospital-based home care agencies and to investigate the effects of long-term care insurance for the elderly. METHOD The subjects were the home care service recipients aged 65 and older in 172 hospital-based, home care agencies registered in Health Insurance Review & Assessment Service in January, 2007. The data were collected using a questionnaire from March 16 to April 15, 2007. The questionnaire return rate was 43.8%. RESULT The hospital-based home care agencies were able to visit 66.5% of the national administrative districts. Of the home care service recipients, over 50% were 65 years old and older. About 43% of the agencies reported that over 50% of their patients would be subject to the long-term care insurance. They expressed concern that home care services would be withdrawn once the insurance system is initiated. CONCLUSION This study suggests that hospital-based home care agencies need to manage home care services with long-term care insurance. It also recommends developing guidelines for the use of services and referrals.
PURPOSE The study was to get basic data on the well-being of middle-aged people concerning their preparation for their upcoming old age and their quality of life. METHODS The subjects were 440 people aged between 40 and 59 living in Seoul and Gyeonggi-do. RESULTS First, physical preparation for old age was affected by religion, children, health, monthly income, and economical status, and these factors were shown statistically significant. Emotional preparation and spiritual preparation were also related to the above-mentioned factors. Secondly, in the subjects' mental picture, a nursing home was a place for the aged without anyone who is going to take care of them and without abilities to care of themselves. Preference for nursing home was based on two factors, i.e., nursing and treatment care, and the cost was about one million won per month. Also they wanted that the government should pay a certain amount for private users. CONCLUSION Therefore, people should lead a life of good health-related habits along with economical preparation for their old age.
PURPOSE To examine factors affecting long-term care hospital patients' intention of transfer to a nursing home. METHOD A questionnaire survey was conducted in Aug. 2007 that included 655 patients from 49 long-term care hospitals. The survey aimed to assess the patients' health status, family status, cost and intention of transfer to a nursing home. Institutional characteristics were analyzed from the nationwide database of Health Insurance Review & Assessment Service. The affecting factors were examined by employing chi-square test and logistic regression using SAS 8.2. RESULT Of the subjects, 32.4% had intention of transfer to a nursing home. The intention of transfer to a nursing home was affected by moderate or severe pain, living together with the primary carer, high cost uncovered by insurance, and recognition of nursing home. CONCLUSION For appropriate service utilization, a higher level of care is needed to satisfy patients at nursing homes and a balanced fee schedule is needed between long term care hospitals and nursing homes. It is desirable to encourage transfer to a nursing home at which nurses support patients and their families by giving information, coordination, and to make efforts to establish a reference system.
